We’ve welcomed artificial intelligence into our project workflows with open arms. It drafts our reports, analyses our data, and answers our most complex questions in seconds. We trust it. But what if that trust is fundamentally misplaced? What if the AI you rely on for critical project decisions has a built-in incentive to lie?


This isn’t a dystopian fantasy. It’s a phenomenon known as “AI hallucination,” and it’s one of the most significant and least understood risks in the modern project environment. When a large language model (LLM) “hallucinates,” it doesn’t just get something wrong; it confidently and plausibly presents fabricated information as fact. It’s not a bug; it’s a feature of how these systems are designed, and understanding this is critical for anyone staking a project’s success on AI-generated output.


What is an AI Hallucination?

When faced with a question it doesn’t know the answer to, the model’s programming encourages it to produce the most probable-sounding sequence of words, rather than admitting uncertainty.

Think about that. The system is optimised for plausibility, not for truth. It has learned from a vast dataset of human text how to sound authoritative and correct. When it encounters the edge of its knowledge, it doesn’t stop. It improvises, weaving a tapestry of fiction that looks and feels like fact. This is why, as IBM notes, hallucinations can be so dangerous: they are “non-sensical or inaccurate outputs” delivered with the full confidence of a machine that we have been conditioned to see as objective.


The Root of the Lie: A System Built on Guesswork

The core of the problem lies in the way these models are trained. They are rewarded for correctly predicting the next word in a sentence. This process makes them incredibly adept at mimicking human language patterns, but it doesn’t equip them with a true understanding of the concepts they’re discussing. Their goal is to complete the pattern, not to verify the information within it.


This creates a fundamental misalignment. A project manager asks an AI for a factual data point for a risk assessment. The AI, lacking that specific data, doesn’t respond with “I don’t know.” Instead, its programming compels it to generate a response that is statistically likely to follow the question. It might invent a statistic, fabricate a source, or misrepresent a real one. The output is grammatically perfect, contextually relevant, and utterly wrong.


This is the critical vulnerability for project delivery. A study from the University of California, Irvine found a dangerous mismatch between our perception of AI reliability and its actual performance. We are predisposed to trust the confident, articulate output of these machines, creating a significant blind spot in our project assurance processes.


The Project Management Blind Spot

How can this manifest in a real-world project? Consider these scenarios:


  • Flawed Risk Assessments: Your team uses an AI to research potential supply chain vulnerabilities. The AI hallucinates a report about a non-existent geopolitical instability, leading you to invest heavily in a mitigation strategy for a risk that isn’t real, while ignoring genuine threats.

  • Inaccurate Stakeholder Reporting: An AI is tasked with summarising project progress. It misinterprets a data trend and confidently reports that a key milestone is ahead of schedule. This false good news is passed on to stakeholders, leading to a disastrous loss of credibility when the truth emerges.

  • Compromised Technical Specifications: A junior engineer uses an AI to find a technical specification for a component. The AI provides a plausible but incorrect value. The error isn’t caught until late-stage testing, forcing costly rework and delays.


In each case, the failure isn’t just a simple error. It’s a failure of trust, amplified by the perceived authority of the AI. As one analysis puts it, these hallucinations stem from the core limitations in the AI’s training data and its probabilistic architecture. They are, for now, an “innate limitation of large language models.”


Building a Framework of Trust

So, can we trust anything? The answer is yes, but not blindly. The solution isn’t to abandon these powerful tools, but to wrap them in a robust framework of human oversight and critical thinking. Project managers must become expert interrogators, not just passive consumers, of AI-generated content.


  1. Always Verify: Treat every piece of factual information from an AI as unverified until it has been cross-referenced with a primary source. Insist that the AI provides clickable source links, and then actually click them.

  2. Human-in-the-Loop: Implement workflows where AI-generated content is always reviewed by a human expert before it is used for decision-making. The AI is the junior analyst; the human is the seasoned manager who signs off on the work.

  3. Pressure-Test Outputs: Actively challenge the AI’s conclusions. Ask it for alternative interpretations, for the data that contradicts its findings, or to argue the opposite case. This can often reveal the brittleness of a hallucinated response.
